This stage is called lactogenesis I. And after delivery, lactogenesis II will arrive, with an abundant production of <strong>breast milk<\/strong>, popularly known as &#8220;<strong>milk coming in<\/strong>.\u201d And finally, there will be galactopoiesis or lactogenesis III, which is the process that maintains the production of <strong>milk<\/strong> once <strong>breastfeeding<\/strong> is established.   <\/span><\/p><p><span style=\"font-weight: 400;\">Unlike other mammals, lactogenesis II or <strong>milk coming in<\/strong> begins a few days after delivery. These days represent a critical period for <strong>breastfeeding<\/strong>, as this is when many of the difficulties with breastfeeding originate. <\/span><\/p><h3 class=\"wp-block-heading\">Regulation of milk synthesis, secretion, and expulsion<\/h3><p><span style=\"font-weight: 400;\">Although the maternal hormonal changes that occur postpartum trigger the <strong>milk coming in<\/strong>, frequent sucking and emptying of the breast are also important. When we talk about the production and ejection processes, several regulatory systems are at play, all strongly influenced by the baby&#8217;s sucking and other stimuli on the nipple.  <\/span><\/p><h4 class=\"wp-block-heading\">Prolactin<\/h4><p><span style=\"font-weight: 400;\">During pregnancy, in response to increased levels of estrogen and progesterone, the pituitary gland produces prolactin. This stimulates <\/span><b>milk production<\/b><span style=\"font-weight: 400;\">. However, during pregnancy, it remains inactive because progesterone and placental lactogen prevent prolactin from binding to its receptors (competitive binding). After delivery, the levels of these two hormones decrease rapidly, and the action of prolactin is no longer blocked, initiating breast milk secretion.<\/span><\/p><p><span style=\"font-weight: 400;\">After delivery, prolactin release occurs in pulses after various stimuli, the most effective being nipple sucking. The maximum peak of plasma prolactin concentration occurs approximately 20-30 minutes after sucking, so its most important effect is to produce <strong>breast milk<\/strong> for the next feeding. Hence the saying, &#8220;<strong>the more sucking, the more production<\/strong>.&#8221; This effect is of particular importance when <strong>breastfeeding<\/strong> is being established, since after a few weeks there is no relationship between the amount of prolactin and the amount of <strong>breast milk<\/strong> produced.   <\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-cfa858f elementor-align-center elementor-widget elementor-widget-button\" data-id=\"cfa858f\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"button.default\">\n\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-button elementor-button-link elementor-size-md elementor-animation-grow\" href=\"https:\/\/happymami.com\/en\/customize-your-happymami-bottle-in-4-steps\/?goto=83588\">\n\t\t\t\t\t\t<span class=\"elementor-button-content-wrapper\">\n\t\t\t\t\t\t\t\t\t<span class=\"elementor-button-text\">Start designing your bottle<\/span>\n\t\t\t\t\t<\/span>\n\t\t\t\t\t<\/a>\n\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b17a415 elementor-widget elementor-widget-text-editor\" data-id=\"b17a415\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<div class=\"wp-block-buttons\" style=\"border-style: none; border-width: 0px; border-radius: 0px; padding: 0;\"><div><img decoding=\"async\" src=\"https:\/\/happymami.com\/wp-content\/uploads\/2025\/09\/blog-protege-lactancia-reproduce-anatomia-v2.webp\"><\/div><\/div><h4 class=\"wp-block-heading\">Oxytocin<\/h4><p><span style=\"font-weight: 400;\">Oxytocin is released in response to nipple stimulation, by sucking or manipulation; and in response to pleasant physical, visual, auditory, or emotional stimuli, generally related to the <strong>baby<\/strong>.<\/span><\/p><p><g id=\"gid_0\">This hormone is responsible for the contraction of the cells that surround the alveoli, producing<\/g><g id=\"gid_1\"> milk <\/g><g id=\"gid_2\">ejection<\/g><g id=\"gid_3\">, facilitating the movement of <\/g><g id=\"gid_4\">breast milk<\/g> through the ducts to the nipple.<\/p><p><span style=\"font-weight: 400;\">This process occurs continuously during sucking and refills the ducts as they empty. <\/span>It is important to keep in mind that stress, anxiety, and other sudden unpleasant physical stimuli can block the oxytocin ejection reflex.<\/p><h3 class=\"wp-block-heading\"><b>Other regulatory factors<\/b><\/h3><p><span style=\"font-weight: 400;\">The retention of <strong>breast milk<\/strong> inside the alveoli increases intra-alveolar pressure, affecting the function of the alveolar cells. It can also compromise vascularization and hinder hormonal arrival, slowing down <strong>milk production<\/strong> when the breast is full or there is no effective drainage of the gland.<\/span><\/p><p><span style=\"font-weight: 400;\">Your <strong>baby<\/strong> cancels these inhibitory effects by adequately emptying your breast. In this way, the production of <strong>breast milk<\/strong> adapts to the needs of your <strong>baby<\/strong>, from one feeding to another and independently for each breast. <\/span><\/p><h3 class=\"wp-block-heading\"><b>Lactogenesis II: Milk coming in <\/b><\/h3><p><span style=\"font-weight: 400;\">As we mentioned, the beginning of <\/span><b>lactogenesis II or \u201cmilk coming in\u201d<\/b><span style=\"font-weight: 400;\"> occurs after the sharp postpartum drop in progesterone and placental lactogen. During this phase, the synthesis of the components of <strong>breast milk<\/strong> increases, causing the breasts to increase in size and be warm and painful. There is no relationship between the intensity of the symptoms and the subsequent production of <strong>breast milk<\/strong>.<\/span> <\/p><p><span style=\"font-weight: 400;\">Clinically, the \u201c<strong>milk coming in<\/strong>\u201d occurs around the <\/span><b>third day<\/b><span style=\"font-weight: 400;\"> postpartum, but up to 25% of mothers perceive the milk coming in after 72 hours.
